Supporting Living Services Waiver (SLS)
The Supported Living Services Waiver (SLS) is for people who are very independent or people who have lots of natural or community support. The person on this waiver lives with family or in their own home and wants to add services to their Medicaid so they can continue to do so.
There is no waitlist for this waiver.

Who is eligible for services?
To be eligible for the SLS waiver you must:

Be at least 18
-
Have a Developmental Disability Determination
-
Need support with activities of daily living
-
Have Long Term Care Medicaid by meeting the Social Security disability definition & financial requirements for Colorado Medicaid
